Having paperwork completed ahead of time allows us to maximize your session time. A New Client Information Form and a Disclosure Form are required before services can be provided. The New Client Information Form provides basic contact information and should be completed prior to the initial assessment and brought to the assessment appointment.
A Disclosure Form outlines the Client and Counselor Rights and Responsibilities and information about the counseling process; it is required to be completed prior to the onset of the first appointment.
The Release of Information Form is only required when the client desires for the counselor to provide information to a third party.
The Group Member Form must be completed prior to participation in therapeutic phone groups.
